How it Works

With COMMpanion, you are not just enrolling in a healthcare service. You are establishing a trusted relationship with a dedicated primary care provider who gets to know you and your health needs, just like a traditional family practice. As an added benefit, our patient portal gives you direct access to nurses and other healthcare professionals whenever you need support.

Build your Health Plan

From your very first visit, your provider takes the time to understand your full medical history, current concerns, and personal goals. Together, you will develop a thoughtful, individualized care plan that reflects not only clinical priorities, but also what matters most to you. This plan becomes a shared roadmap, guiding care decisions with clarity and continuity throughout the year.

Proactive Care Management

Rather than waiting for problems to arise, your care team remains actively engaged in your health. They monitor your progress, anticipate your needs, and are available at the right moments—whether for preventive screenings, follow-up care, or adjustments to your plan. In collaboration with specialists when needed, each step is thoughtfully managed, reducing friction and ensuring nothing is overlooked. The result is a more connected, responsive experience in which you feel known, supported, and consistently cared for by a provider who is present not just at visits, but across the full arc of your health.

Sick and Well Visits

You may request appointments for urgent care or other concerns at any time. These could be follow-up calls regarding questions on ongoing concerns or newly presenting symptoms.

Prescription refills

When your medication is up for renewal, we will notify you that it is time for a refill, and you can tell us where you would like to fill the prescription. We can even have it sent to your house if you prefer.
