How it Works

At COMMpanion, you develop a relationship with a qualified Primary Care Provider (PCP) just like a traditional family practice, and, through our app, you also have access to an entire team dedicated to your care, including nurses and health navigators.

Build your Health Plan

At your first PCP appointment, your provider will assess your medical history and any current health concerns that you might have. Next, the PCP will create a healthcare plan for the upcoming year that both you and the care team will use to maintain your health.

Proactive Care Management

Using your health plan, our care team will proactively reach out throughout the year to support your journey to good health. As appointments, tests, vaccinations, or other diagnostic needs come up, our health navigators will contact you to schedule those at one of our affiliated facilities.

Sick and Well Visits

You may request appointments for urgent care or other concerns at any time. These could be follow-up calls regarding questions on ongoing concerns or newly presenting symptoms.

Prescription refills

When your medication is up for renewal, we will notify you that it is time for a refill, and you can tell us where you would like to fill the prescription. We can even have it sent to your house if you prefer.

Answers to health questions

Message the care team any time via the app or website with non-urgent questions and receive responses within hours, frequently within minutes.

Help navigating referrals and specialist care

Our care team will assist you with referrals for specialist care, tests, or other in-person appointments. Give us a call at 833-PSH-CMPN (833-774-2676), Monday to Friday, 7 a.m to 7 p.m. (excluding holidays).

Answers

Questions and Answers

  • What is Virtual Primary Care and how would it benefit my organization's employees?
Virtual Primary Care is the same high-quality care that your doctor currently provides but tailored to your schedule and without the trip to the office. Because the PCPs in the COMMpanion Virtual Primary Care network are limited to fewer patients than typical PCPs, they are able to take the time to get to know you and your healthcare needs. Virtual Primary Care is an ideal solution for patients who have: Difficulty reaching their doctor’s office Strict work hours Chronic medical conditions, such as diabetes or heart disease, which could benefit from additional support. Want a personalized approach to their healthcare and a team to assist them in keeping track of their health throughout the year. A busy life juggling multiple responsibilities like childcare, eldercare or simply like the flexibility of accessing care on their terms.
  • Does this replace employee health insurance?
COMMpanion supplements rather than replaces primary health insurance. As part of your membership, you will receive access to a PCP and care team, but it will not include diagnostic work, specialist care, or in-person primary care appointments. You will be responsible for those costs either through insurance or out-of-pocket. We recommend maintaining health insurance to cover surgeries and other medical procedures, but you may be able to move to a lower cost, high deductible insurance plan. Every person's medical situation is unique, and you should consider your own and your family's healthcare needs before making insurance decisions. However, COMMpanion can help cut down on costs by improving your overall health through dedicated and personalized care and by lowering the costs of primary care visits.
  • What if I need to be seen in person?
We have found that approximately 80% of all visits can be conducted virtually. If your provider determines you need to be seen in person, your care team will work with you to schedule an in-person visit that works for your schedule. In that case, we will bill you or your insurance for the cost of an in-person primary care visit.
  • Can I keep my current primary care provider for in-person visits?
In order to prevent disruptions and miscommunication in care, we discourage keeping your current primary care provider when using COMMpanion. Our primary care providers will serve as your main contact for all healthcare needs.
  • How do I get a diagnostic test (e.g.: blood draw or imaging)?
We will schedule lab tests or imaging for you at an affiliated facility and communicate the results to you in a timely way.
  • Can my family join?
Any member of your family 18 years of age and older is welcome to join COMMpanion.
  • How do I access the service?
You may access the COMMpanion service in three ways. COMMpanion App The COMMpanion app is the most convenient way to access care with COMMpanion. You can find it by searching the Apple App Store or Google Play for “COMMpanion”. Here you will find your appointment schedule, summaries of each visit and your health care plan. From the app, you can message our health navigators, nurses and physicians; schedule appointments, including urgent care visits; and request referrals and prescription refills by messaging the care team. You will use either the app or the website to enter the video calls with your doctor during your scheduled appointments. When opening the app for the first time, you will be asked to enter the username and temporary password that were emailed to you when you were enrolled in the program. You will be asked to change the password when you first log in. If you cannot find your login information, check your spam filter or call us at the number below. Website If you prefer to use your computer to access your information and care team, everything that is available in the app is also available here on our website. Your username and password are the same for the app and the website. You may login at the link at the top of the page. Phone If you are having trouble using the app or prefer to discuss your question over the phone, our health navigators and nurses are available Monday through Friday, 7 a.m. to 7 p.m., at 833-PSH-CMPN (833-774-2676).
  • How do I make an appointment?
Log in to the COMMpanion app or website. Click on “Schedule an Appointment”. Instructions for accessing your appointment will be provided after the appointment is made. If you have any questions along the way, simply message a health navigator for assistance.
  • Is my medical information secure?
COMMpanion maintains the same high level of security that all Penn State Health medical teams provide. COMMpanion is HIPAA compliant and meets all other state and federal healthcare security laws. Furthermore, your information is private and will not be sold to a company without your knowledge.
