This information will allow your care team to personalize your experience based on the information you provide, including your medical history, relevant health details, prescriptions and any ongoing concerns.

Fill Out and Submit this Form

Click the link below and complete the Patient Authorization for Release of Medical Records form before your visit. This form will allow your provider to know your medical background more completely by communicating with your previous health care providers.

Scheduling Your First Visit

After completing these steps, you are now ready to schedule your first visit! Login with your user name and password. Choose “Schedule a Visit”, select either “Choose by Date” or “Choose by Provider”, then follow the prompts to schedule your appointment. 

 

If you are having trouble using the app or prefer to discuss your questions over the phone, our health navigators and nurses are available Monday through Friday, 7 a.m. to 7 p.m. (excluding holidays), at 833-774-2676.
